Transitions of Western Illinois
Transitions is a not-for-profit agency that primarily serves Adams County residents. The agency serves people who have a serious mental illness, people who have a developmental disability, and people who have no mental disability, but who are struggling with life problems that can affect anyone. Services include outpatient mental health services for people of all ages, vocational rehabilitation and training services, life skills training services, case management services, developmental services for children ages birth to three, and crisis intervention services. The agency also provides supervised housing (group homes) and supported housing (supported apartments) for adults who have a serious mental illness or a developmental disability.
